This test was developed by National Depression Screening Day (NDSD) Executive Director, Douglas G. Jacobs, M.D. It is not designed to provide an actual diagnosis of depression or any other mental health disorder. For that, you will need a complete evaluation by a mental health or other health care provider.

2. I have trouble doing or enjoying things I used to do.
3. I sleep too little or too much.
4. I feel keyed up and anxious.
5. I worry constantly about everything.
6. I have nightmares or frightening memories about a terrible event that happened in the past.
7. My mood swings between overly “high” and irritable to down and sad.
8. Sometimes my mind races and I don’t need to sleep.
9. I suffer from aches and pains and don’t know why.
10. I think about killing myself.

Keep in mind that symptoms of different disorders often occur at the same time.

If you answered yes to questions 1-3, you may have symptoms of depression.

If you answered yes to questions 4 and 5 you may have symptoms of anxiety.

If you answered yes to questions 4, 5 and 6 you may have symptoms of post-traumatic stress disorder.

If you answered yes to questions 7 and 8 you may have symptoms of bipolar disorder.

If you answered yes to question 9, it is important to remember that many psychological disorders, particularly depression and anxiety, go hand-in-hand with physical aches such as headaches, stomach problems or backaches for which there is no physical cause.

If you answered yes to question 10, seek help immediately regardless of your answers to any other questions.

It is natural to experience some or even all of these symptoms at some points in life. But, if you experience these symptoms for more than two weeks, it indicates that you may need professional help.
